- There are a total of 10 DAW (Dispense As Written) codes that range from 0-9. They were established by the National Council for Prescription Drug Programs (NCPDP), a nationally accredited standards organization. DAW 0: Used for Single Source and Generic Products. DAW 1: Used for a particular Single Source or Multi-Source Branded product that the Physician has specified in accordance with state law. The verbiage used, such as "Brand Medically Necessary" or "Dispense as Written" along with the positioning of the signature lines vary by state prescription format. For Medicaid prescriptions, it is required that "Brand Medically Necessary" be hand written on the prescription by the Prescriber. DAW 2: Used for Multi-Source Brands that may have a generic equivalent available and the patient has requested the Brand be dispensed. The prescription must have "Patient Requested Brand" documented on the hard copy. DAW 5: Used currently for BRanded drugs utilized as a generic. In cases where a generic drug has been reclassified to Multi-Source Brand status the code, DAW 5 should be used instead of DAW 0. DAW 6: Used for specific state Medicaid plans (Lousiana, South Carolina, and Texas) that require, DAW 6 be used in place of DAW 1. Same requirements as DAW 1 apply. DAW 7: Used in specific states on drugs that are listed on state negative formularies. Florida Medicaid uses DAW 7 in lieu of DAW 1. Same requirements as DAW 1 apply for Florida Medicaid. DAW 8: Should only be used after determining that a drug is not available in the marketplace. DAW 9: Used only for Mississippi Medicaid in lieu of DAW 1. Same requirements as DAW 1 apply. Hope that helps.
